How to write an effective online surveys?

In an event where the questions are scaled, it may not give the desired answers like Good, very good, excellent etc. In case of scaled questions it is advisable that the answers are quite balanced and comparable at intervals.

It’s good to have a name to the survey and a brief introduction giving a gist about the survey, so that even they are prepared to take the survey.
